Complete and submit your intake form (available on the forms page) by fax or e-mail. Please note, e-mail is not considered secure but we may use it with your permission.

Free parking is available on site; you may park in any available spot to the side or behind the building. While the office is on the second floor, the building and office is equipped with lifts and an elevator for full wheelchair accessibility.

9AM - 6PM
Weekend appointments are available for certain providers. Same day visits are occasionally available, but please contact us ahead of time. Please provide 48 hour business day notice for cancellations; ie, notice on Monday for a Wednesday appointment, or notice the prior Thursday for a Monday appointment. In the event of a late cancellation or no show, you are liable for the full cost of the visit.
